Job summary

Choice Health at Home is seeking a dedicated Physical Therapist to join our Oklahoma City Home Health team. You will assess patients, create individualized treatment plans, and deliver therapy to improve mobility and independence in the home setting.

Responsibilities include initial evaluations, program development, progress monitoring, caregiver education, and collaboration with the care team, with emphasis on infection control, accurate documentation, and adherence to agency policies for

Qualifications

• Graduation from an accredited physical therapy program and successful completion of the state licensure examination.
• Current, valid Physical Therapy license.
• One year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform and document initial evaluations to identify functional limitations and establish appropriate treatment plans.
• Develop and implement individualized therapy programs to improve movement and restore function.
• Monitor patient progress and adjust treatment plans to achieve best outcomes.
• Educate patients and caregivers on exercises and safe mobility to support independence.
• Collaborate with patients, families, and healthcare team to ensure continuity of care.
• Follow infection control practices and universal precautions during patient care.
• Complete accurate documentation within company timelines.
• Support agency policies and commitment to quality care.

Education

Graduation from accredited physical therapy program; state licensure
