Advocate Health is seeking a Physical Therapist Assistant (PTA) for Home Health coverage in the Chicago South & Southwest service area. The role supports a Monday–Friday, 8:00 AM–5:00 PM schedule in a home-based setting with a $5,000 sign-on bonus for external candidates.

The PTA will work under a supervising therapist, delivering high-quality, patient-centered care, and ensuring compliant documentation and care plans. Illinois licensure is required or eligibility to obtain licensure.

Formación

• 2 years of clinical experience or demonstrated clinical affiliation experience.
• Illinois Licensed Physical Therapist Assistant or eligible for licensure.
• Proficient in documenting to support reimbursement and regulatory requirements.
• Able to travel within the service area and work a day shift.

Responsabilidades

• Develops restorative relationships with patients throughout the care continuum.
• Follow plan of care under supervising therapist and adjust as needed.
• Promote patient/family knowledge and confidence for self-care and safety.
• Document thoroughly to support reimbursement and regulatory requirements.
