Job summary

Your Health is seeking a skilled Primary Care Physical Therapy Assistant to join our team serving homebound patients in South Carolina. You will work with licensed PTs to implement treatment plans, guide exercises, and monitor progress in homes, ALFs, and ILFs.

This is a full-time, salary-based role offering a monthly bonus opportunity, competitive compensation, and benefits. You will collaborate with facilities and families to deliver compassionate care and coordinate discharge planning.

Qualifications

• Associate degree in Physical Therapy Assistant is required.
• Current licensure or eligibility for licensure in the state of practice.
• CPR certification is required.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Guide patients through therapeutic exercises and monitor progress.
• Educate patients on rehabilitation techniques and home exercise programs.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate discharge plans and follow-up services with care teams.
