Job summary

Grace Reliant Health Services LLC-1 seeks an experienced Physical Therapist Assistant to provide home-based PT services in the Perryville region. The TA will work intermittently according to physician orders, delivering skilled PTA care in patients’ homes.

Responsibilities include implementing care plans, instructing patients, maintaining documentation, and ensuring patient safety and progress while applying current research and best practices.

Qualifications

• Must have active license to practice as a PTA.
• Experience delivering home-based physical therapy.
• Ability to instruct patients in exercises and activities.

Responsibilities

• Implement components of care and interventions competently.
• Instruct and counsel patients in exercises.
• Identify patient needs with empathy and adapt treatments.
• Recommend modifications to improve safety and progress.
• Apply research related to restoring function and reducing symptoms.
• Complete documentation for delivery of PT services.
• Collect data and obtain accurate information.
• Measure performance and report in medical records.
• Participate in patient status judgments.
• Adhere to ethical and legal standards.
• Maintain clinical competency and updated knowledge.
