Job description

We're hiring for a full-time Physical Therapist to join our passionate team in Worcestor!

  • Coverage areas include Worcester, Holden, Sterling, Lancaster.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist helps develop a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities
  1. Evaluate the patient's functional status, body systems, and physical therapy needs. Consult with the physician to develop the therapy plan of care.
  2. Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  3. Observe and report patient responses and condition changes to the nurse supervisor and/or physician.
  4. Treat patients to relieve pain, restore or maintain function, using evidence-based techniques.
  5. Educate the healthcare team, patient, and family/caregivers regarding the plan of care, limitations, and progress.
  6. Visit patients within the service area and nearby agencies as needed.
  7. Complete progress notes for each visit and submit them per policy.
  8. Conduct evaluations, develop the PT plan of care within state guidelines, and report outcomes within 24 hours.
  9. Participate in staff meetings and committees as requested.
  10. Supervise home health aides and PTAs as permitted by state regulation and agency policy.
  11. Participate in weekend rotations as needed.
  12. Prepare discharge summaries or patient condition reports every 60 days.
  13. Provide in-service education as required.
  14. Adhere to LHC Therapy Standards of Practice.
  15. Maintain current clinical knowledge and licensure through ongoing education.
  16. Perform other duties as assigned.
Education and Experience

Licensure Requirements

  1. Current Physical Therapy license in MA.
  2. Current CPR certification.
  3. Valid driver's license, vehicle insurance, and access to a dependable vehicle.
