Job summary

CHRISTUS HomeCare - St. Michael, part of LHC Group, seeks a Physical Therapist to assess patients in home health, develop and implement plans of care, and measure progress toward mobility and functional goals.

Licensure in the state is required and a valid driver's license with access to a vehicle is necessary. CPR certification is preferred; competitive total cash compensation and comprehensive benefits are offered.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports patient responses to treatment to the supervising nurse/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
