Job summary

CHRISTUS HomeCare, part of LHC Group within the Optum family, seeks a Physical Therapist in Home Health to assess mobility, endurance, and pain, and to design an effective plan of care.

You will implement interventions to improve function, document progress, and collaborate with physicians and the health care team while following state guidelines.

Requirements: current PT licensure in the state, a valid driver’s license, and CPR certification preferred within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transport.
• CPR certification preferred or able to obtain within 90 days.

Responsibilities

• Evaluates functional status and therapy needs with the physician.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care, limitations and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Develops the PT plan of care within state guidelines.
• Reports outcomes and projected frequency of care.
• Works with diverse patients regardless of age, race or background.
