Enhabit Home Health & Hospice in Alabama is seeking a licensed Physical Therapist to provide skilled care in the home setting. You will evaluate mobility, design treatment plans, and support patients to improve daily living activities and independence.

Join a collaborative team with specialty programs such as Balance and Fall Prevention and Total Shoulder/Hip programs. Must have a valid state driver’s license and reliable transportation; driving is required in this role.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Physical Therapists examine and treat patients with physical impairments using physical modalities to improve mobility and daily living.
• Administers skilled care per physician orders and plan of care under supervision of Branch Director.
• Works with family and health-care team to support patient independence.
• Participates in Specialty Programs like Balance and Fall Prevention, Spine Safety, and joint replacement programs.
