Job summary

Qualifications
- Must have a valid state driver’s license.
- Must possess automobile liability insurance.
- Must wear seatbelts at all times while driving.
- Must have dependable transportation kept in good working condition.
- Must be able to drive an automobile in all types of weather conditions.
- Must be currently licensed in the state of employment.
Responsibilities
- Provide skilled physical therapy services to patients in home health settings.
- Utilize therapies and modalities to improve mobility and self-care in daily living.
- Collaborate with families and health-care team under physician orders and care plans.
- Report progress and coordinate with Branch Director as required.
