Job summary

UnitedHealth Group is seeking a licensed Physical Therapist to provide home health services under physician-directed plans of care. Responsibilities include comprehensive assessments, plan development, and ongoing reassessments within an interdisciplinary team.

Requirements include a CAPTE-accredited PT program, current state license, CPR certification, and 1+ year of clinical experience with care coordination. A valid driver’s license and reliable transportation are needed.

Qualifications

• Current, unencumbered physical therapy license in the assigned state.
• Active CPR certification.
• 1+ year of physical therapy experience; or less than 1 year with residency/mentorship.
• 1+ year of experience in clinical assessment, documentation, and care coordination.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized PT plans of care with physicians.
• Provide skilled PT interventions and adjust treatment plans per progress.
• Perform ongoing reassessments to support care planning and coordination.
• Collaborate with interdisciplinary teams to communicate patient progress.
