Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to assess functional status, mobility, endurance, and pain, and to develop treatment plans to improve patient well‑being. You will provide interventions and evaluate progress in partnership with the care team.

Responsibilities include evaluating patients, planning care, documenting visits, and coordinating with physicians and nurses. Requires current PT licensure and a valid driver’s license; CPR certification preferred or

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
