Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to evaluate patient needs, develop and implement therapy plans, and monitor patient progress in home settings. The role emphasizes coordination with physicians and nursing staff to optimize mobility and function.

Responsibilities include conducting assessments, establishing care plans, delivering interventions, educating patients and families, and ensuring timely documentation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Evaluates the patient's functional status and needs; assists in developing the plan of care.
• Provides PT services within state scope and plan of care; coordinates with health care team.
• Observes and reports patient responses and changes to supervisor/physician.
• Treats patients to relieve pain and improve function; maintains standards of practice.
• Educates patient and family about plan of care and progress toward goals.
• Visits as per Plan of Treatment; documents progress notes.
• Completes evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes and projected frequency of care.
