Compassus is seeking a licensed Physical Therapist to provide home-based therapy services in California. You will evaluate patients, develop treatment plans, implement therapies, and discharge plans in collaboration with a multidisciplinary team.

Responsibilities include comprehensive assessments, planning and administering PT programs, documenting progress, and coordinating with physicians. Bachelor's degree in Physical Therapy required; 1 year adult rehab preferred.

Qualificações

• Bachelor's degree in Physical Therapy from an approved program.
• Preferred: 1 year therapy with adult physical disabilities.

Responsabilidades

• Performs comprehensive patient assessments including ROM, manual muscle testing, gait and functional analysis to establish/update treatment plans.
• Plans and administers physician-ordered physical therapy programs to restore function and relieve pain.
• Records and evaluates assessment findings, treatment plans and progress notes to guide care and meet regulatory requirements.
• Administers therapeutic exercises and activities using equipment and assistive devices.
• Provides treatments involving physical agents as prescribed.
• Instructs and motivates patients and families in safe use of therapy equipment and exercises at home.
• Collaborates with physicians and interdisciplinary team members to coordinate care and adjust treatment programs.
• Identifies need for referrals to additional disciplines and facilitates coordination of services.
