Job summary

Providence at Home with Compassus seeks a Physical Therapist to provide home health therapy, from evaluation through discharge, in a multidisciplinary team setting. You will implement physician-ordered plans and coach patients and families for safe home exercises.

The role requires a state PT license and a bachelor’s degree, with preference for 1 year of adult therapy experience. A $10K sign-on bonus is available; travel may be required in the service area.

Qualifications

• Bachelor's Degree in Physical Therapy (state-approved).
• Current, unencumbered Physical Therapy license required within 30 days of hire.
• 1 year therapy with adults preferred.

Responsibilities

• Performs comprehensive patient assessments and develops individualized treatment plans.
• Plans and administers physician-ordered PT programs to restore function and relieve pain.
• Records progress and modifies plans to meet regulatory and care standards.
• Supervises assistants and aides to ensure quality and compliance.
• Collaborates with physicians and the care team to coordinate therapy in home settings.
