Compassus is seeking a Physical Therapist to provide home health evaluation, treatment planning, and therapy implementation in California. You will work with clients and families in their homes, coordinating care with a multidisciplinary team.

The role involves discharge planning, supervising PT aides, and ensuring regulatory compliance. A current PT license and BLS within 30 days are required, with travel within the service area expected.

Qualifications

• Bachelor’s degree in Physical Therapy from an approved program.
• Current, unencumbered Physical Therapist license.
• BLS certification within 30 days of hire; CA license required upon hire.

Responsabilités

• Perform comprehensive patient assessments (ROM, gait, functional analysis) to establish/update treatment plans.
• Plan and administer physician-ordered PT programs to restore function and prevent disability.
• Record and evaluate assessment findings, plans, and progress to guide care and meet regulatory requirements.
• Administer therapeutic exercises and activities using equipment and assistive devices.
• Provide treatments (heat/cold, massage, and other modalities) as prescribed.
• Instruct patients and families on safe use of therapy equipment at home.
• Collaborate with physicians and team members to coordinate care and adjust plans.
• Identify referrals to other disciplines and facilitate service coordination.
• Supervise PT assistants and aides to ensure quality and compliance.
