Job summary

UnitedHealth Group is seeking a Physical Therapist to deliver skilled PT services in patients' homes across California. You will promote mobility, restore function, improve strength and balance, and enhance safety and independence in the home setting.

Collaborating with patients, caregivers, and an interdisciplinary team, you will conduct 30-day reexaminations, plan discharges, and participate in a comprehensive benefits program. A $10,000 sign-on bonus is available for external candidates.

Qualifications

• Current license to practice physical therapy in the state of assignment.
• Graduate from CAPTE‑accredited physical therapy program.
• At least 1 year of PT experience in home health care; mentoring/residency if <1 year.
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy services at patients' homes to improve mobility and independence.
• Conduct reexaminations every 30 days and perform discharge planning.
• Collaborate with patients, caregivers and an interdisciplinary care team.
