We are hiring for a Physical Therapist/PT to join our passionate team in Scranton, PA!

Responsibilities
  • Evaluate patient's functional status, body systems, and physical therapy needs; collaborate with physicians on the plan of care.
  • Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and coordinate with the healthcare team.
  • Monitor and report patient responses and condition changes to the nurse supervisor and/or physician.
  • Implement treatment plans to relieve pain, restore, develop, or maintain maximum functioning using evidence-based techniques.
  • Educate patients, families, and healthcare team members about the plan of care, limitations, and progress.
  • Visit patients across the service area and nearby agencies as needed.
  • Complete and submit progress notes per policy after each visit.
  • Develop evaluation and treatment plans within state guidelines; report outcomes and projected care frequency promptly.
  • Participate in staff meetings and committees as required.
  • Supervise home health aides and PTAs as permitted by law and policy.
  • Participate in weekend rotations to provide services as needed.
  • Prepare discharge summaries and patient condition reports regularly.
  • Provide in-service education upon request.
  • Adhere to therapy standards and maintain current clinical knowledge and licensure.
  • Perform other duties as assigned.
Education and Experience
Licensure Requirements
  • Current Physical Therapy license in the state of practice.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and reliable transportation.
