Responsibilities include developing plans, documenting progress, and coordinating with the health care team in a home setting. Requirements include current Arkansas PT licensure, CPR certification, and reliable transportation.

Qualifications

• Current Physical Therapy licensure in Arkansas.
• Current CPR certification.
• Valid driver’s license and access to a dependable vehicle or reliable public transportation.

Responsibilities

• Evaluates the patient’s functional status and PT needs, assisting in developing the plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes patient response to treatment and reports changes to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and care team about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and anticipated care frequency.
