Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to a dependable vehicle.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs as part of developing the plan of care.
• Performs interventions to improve and enhance patient well-being within scope of practice.
• Observes and reports patient response to treatment to supervisors and physicians.
• Instructs patients, families, and care teams regarding plan of care and progression toward goals.
• Completes progress notes for each visit and documents outcomes.
