Sun Recruiting, Inc. is seeking a passionate Physical Therapist to deliver in-home health care across Colorado communities. This full-time role focuses on patient-centered rehabilitation and improving mobility in patients' homes.

You will conduct assessments, develop and implement customized treatment plans, guide therapeutic exercises, educate families, document progress, and collaborate with a multidisciplinary team while maintaining regulatory standards.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Master’s degree in physical therapy approved by CAPTE.
• Currently licensed to practice as a physical therapist within the state of operation.
• CPR certification required.
• Must be a licensed driver able to travel to all locations.

Responsabilités

• Perform comprehensive physical assessments and analyze medical histories.
• Create and implement tailored treatment plans for rehabilitation goals.
• Conduct therapeutic exercises and training programs to improve mobility.
• Educate patients and families about injury prevention, wellness, and recovery.
• Monitor progress and adjust treatment plans to achieve goals.
• Collaborate with a multidisciplinary healthcare team.
• Maintain patient records and documentation per regulations.
