Job summary

CareerNerds, LLC seeks a dedicated Physical Therapist to join our home health care team in Colorado. This full-time role centers on delivering patient-focused therapy in patients' homes, promoting mobility and independence through personalized treatment plans.

You will perform assessments, develop rehabilitation goals, educate families, document progress, and collaborate with a multidisciplinary team to ensure safe, effective care in a dynamic home-based setting.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program.
• Currently licensed to practice in the state of operation and CPR certified.

Responsibilities

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
• Create and implement tailored treatment plans addressing rehabilitation goals and patient needs.
• Conduct therapeutic exercises and training programs to improve mobility and functionality.
• Educate patients and families about injury prevention, wellness strategies, and recovery journey.
• Monitor and document patients' progress, adjusting treatment plans as necessary.
• Collaborate with a multidisciplinary healthcare team for holistic patient care.
• Maintain meticulous patient records and documentation in compliance with standards.
