Job summary

CenterWell Home Health, part of Humana Inc., seeks a Home Health Physical Therapist to deliver skilled PT at patients' homes, focusing on rehabilitation and functional improvement. You will work with a multidisciplinary team to restore independence and improve quality of life.

Responsibilities include developing treatment plans, supervising staff, and documenting patient progress. Moderate travel is required, with a non-remote work arrangement and competitive pay and benefits.

Qualifications

• Degree from an accredited Physical Therapy Program (APTA approved).
• Current, unrestricted Physical Therapy license required.
• CPR certification current and active.
• Strong organizational and communication abilities.
• Prior home health or pediatric experience a plus.

Responsibilities

• Plan and administer prescribed skilled physical therapy for home care patients.
• Screen and test patient strength to guide treatment and set goals.
• Develop rehabilitation programs aligned with physician plans of care.
• Collaborate with physicians and team to adjust therapy as needed.
• Supervise therapy assistants and home health aides, monitor documentation.
• Document observations, interventions, and progress and notify care team.
• Report patient progress to physician and care management staff.
