Job summary

CenterWell Home Health, a Humana company, is seeking a Home Health Physical Therapist to help patients regain strength and independence in their homes. You will plan and administer PT care, assess progress, and guide rehabilitation to achieve optimal function.

You will supervise PTAs and aides, document care meticulously, and report progress to the care team and family, ensuring coordinated, patient-centered treatment across the home health setting.

Qualifications

• Degree from an accredited Physical Therapy program (APTA approved).
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with physician plans and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician, Clinical Manager, care team, patient, and family; submit evaluations, plans, progress notes, and discharge summaries.
