Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home physical therapy, advancing patients' strength, mobility, and independence. You will report to the Clinical Manager and work within Humana's CenterWell network to provide personalized care in a patient home setting.

The role requires an unrestricted PT license and CPR certification, with prior collaboration in healthcare teams.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed PT in patients' homes to maximize function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehab programs aligned with physician plans.
• Provide clear instructions to PTAs and Home Health Aides and supervise them.
• Document observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient, and family; submit all required documents.
