Job summary

CenterWell Home Health, a Humana Inc. company, seeks a Home Health Physical Therapist to deliver in-home care focused on functional improvement and independence. You will report to the Clinical Manager and work to enhance patients' quality of life in their homes.

The role requires an active Physical Therapy license, CPR certification, and experience collaborating with a healthcare team; preferred one year of PT experience.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to attain the highest functional level.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehab programs aligned with physician plans.
• Provide PTAs and Home Health Aides with clear care instructions and supervision.
• Document observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team and family; submit required records.
