Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs per plan of care.
• Provides PT services within scope of practice and in coordination with the health care team.
• Observes and reports patient responses and changes in condition to supervisor/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family on plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes, goals, and frequency of care.
