Job summary

UnitedHealth Group’s DFW Home Health division is seeking a Physical Therapist to assess and treat patients in their homes, developing and implementing therapy plans to improve functional status, mobility, and comfort. You will work with a care team to monitor progress and ensure quality outcomes.

The role requires current PT licensure, a valid driver’s license and reliable transportation, and may involve CPR certification. Competitive pay and a comprehensive benefits package are included.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and PT needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient’s response to treatment and any changes to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients, family, and caregivers about the plan of care and progress toward goals.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
