Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess and evaluate patient needs, determine treatment plans, and implement interventions to improve mobility and well-being. You will collaborate with physicians and the care team to ensure high-quality, patient-centered care in home settings.

The role features a blended compensation structure with per-visit and hourly pay, along with a comprehensive benefits package.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning using standards of practice.
• Instructs patients and family/caregivers regarding the plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
