Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess patients’ functional status, plan and implement therapy, and monitor progress in a home-based setting. You will collaborate with the care team to improve mobility and quality of life for diverse patients.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment and any changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes.
