Job summary

ABL Health Care in Lafayette, CA is seeking a dedicated Full Time Physical Therapist (PT) to join our Home Health team. You will assess patients, develop and execute care plans, and monitor progress in patients' homes.

Qualifications

• Current PT license to practice in California.
• Experience in Home Health or Hospice care preferred.
• Willingness to travel within a 15–25 mile radius.

Responsibilities

• Provide and oversee skilled intermittent care to patients in their homes.
• Evaluate and re-evaluate patient status regularly.
• Administer treatments and monitor patient conditions.
• Develop and revise the patient care plan (POC).
• Maintain accurate clinical records using a company device.
• Coordinate with physicians, case managers, and nursing leadership.
