Job summary

Trinity Health At Home is seeking a licensed Physical Therapist to deliver high-quality PT services in patients’ homes. You will assess conditions, develop and adjust treatment plans, and implement therapies while educating patients and families.

• Current state license to practice as a Physical Therapist.
• Minimum of one year of experience or completion of a 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.
