Trinity Health At Home – Muskegon (Michigan) is hiring a Home Health Physical Therapist to provide skilled, one-on-one therapy in patients' homes, helping them regain strength, mobility and independence.

The role supports top-of-license practice, autonomy, and meaningful 1:1 patient relationships, with a fast, respectful hiring process and comprehensive benefits starting Day One.

Qualifications

• Current licensure as a Physical Therapist in the State of Michigan.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care preferred).
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide skilled, one-on-one therapy in patients' homes.
• Assess patient needs and develop individualized treatment plans.
• Deliver evidence-based interventions and educate patients and caregivers.
• Support safe recovery at home and coordinate care with clinicians.

As a Home Health Physical Therapist, you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain strength, mobility and independence.
