Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full-time staff position in Spencer, North Carolina. The role serves adult and geriatric clients in Senior Living Facilities on a per-visit basis, primarily in the Salisbury and Lexington areas of Rowan and Davidson Counties.

As part of a multidisciplinary team, you will perform home visits, conduct mobility and function assessments, and develop individualized care plans while documenting clinically in a modern EMR system.

Qualifications

• North Carolina PT license required.
• Minimum of one year of recent work experience as Physical Therapist.
• Graduate from an American Physical Therapy Association approved program.

Responsibilities

• Make home visits to clients in Salisbury/Lexington territories.
• Perform mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans including therapeutic exercises.
• Document observations and interventions in EMR.
• Educate clients and families on rehabilitative care and activities.
