Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a PRN role, providing home health visits to adult and geriatric clients in Salisbury Senior Living and surrounding communities in Catawba County, North Carolina.

The PT will join a multidisciplinary team, delivering rehabilitative care and skilled nursing to help clients stay at home. Prior home health experience and NC licensure are required.

Qualifications

• Current North Carolina PT license.
• Minimum of one year of PT experience.
• Graduate from an APTA/CAAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, balance, strength, safety).
• Implement wound care interventions using evidence-based dressings and topicals.
• Develop and implement individualized care plans with therapeutic exercises and gait training.
• Assess and revise the PT care plan in coordination with nursing.
• Educate clients and families in rehabilitative care for health and independence.
• Accurately document observations and interventions using EMR/tablet.
