Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a PRN opportunity delivering home health visits through our Salisbury Senior Living office in Catawba County, NC.

You will work as part of a multidisciplinary team, assess function, develop individualized plans, educate clients and families, and document care using EMR on a mobile device. This role requires compassionate care and strong communication with patients, families, and other providers.

Qualifications

• Current North Carolina PT license
• Minimum of 1 year PT experience
• Ability to work independently and manage time effectively
• Strong interpersonal skills and problem-solving abilities

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess mobility, gait, balance, strength, and safety.
• Develop and revise individualized care plans with manual therapy and exercise.
• Educate clients and families on rehabilitative care and home exercises.
• Document observations and interventions using EMR on a tablet.
• Coordinate with nursing and multidisciplinary team to optimize care.
• Maintain accurate and complete client records and progress notes
