Qualifications

Responsibilities

• Make home visits to adult and geriatric clients in designated NC counties.
• Perform diagnostic tests and mobility/strength assessments.
• Develop and implement individualized care plans.
• Educate clients and family members on rehabilitative care.
• Document observations and interventions using EMR.
