Job summary

CareCo Medical Home Health seeks a licensed Physical Therapist to provide skilled in-home therapy services in the community. The role emphasizes independent work within an interdisciplinary team and coordination with nurses and other professionals.

The successful candidate will perform home-based PT evaluations, develop Plans of Care, document outcomes, and educate patients and caregivers to support safe recovery. A valid CT license and at least three years of healthcare experience are expected.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Connecticut.
• Minimum of three years of healthcare experience.
• Home health and OASIS experience preferred.
• Strong communication and documentation skills.
• Ability to work independently with minimal supervision.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Perform home-based PT evaluations, assessments, and treatments.
• Complete initial and comprehensive assessments, including OASIS, within required timeframes.
• Develop, implement, and update individualized Plans of Care.
• Document visits and outcomes in a timely manner; communicate changes to care team.
• Provide patient and caregiver education to promote recovery and safety.
• Participate in discharge planning and interdisciplinary care coordination.
• Supervise Physical Therapy Assistants per guidelines.
• Promote safety and comply with standards.
