Job summary

CareCo Medical Home Health seeks a licensed Physical Therapist to provide in-home care within Connecticut. The role involves evaluating patients, coordinating with a multidisciplinary team, and delivering therapy in a home setting.

The position requires a valid CT PT license, reliable transportation, and willingness to travel across designated counties. Medicare experience is preferred, with competitive benefits and professional development opportunities.

Qualifications

• CT license as a Physical Therapist.
• Valid driver’s license and reliable transportation.
• Willing to travel within designated CT counties.
• Strong communication and interpersonal skills.
• Home health experience including Medicare preferred.

Responsibilities

• Assess, evaluate, and provide care for patients in a home care setting.
• Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
• Teach supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
