Job summary

Carilion Clinic is seeking a dedicated Physical Therapist for Home Care in Rocky Mount, VA. The role focuses on interpreting physician referrals, assessing patients, and planning individualized therapies in line with age and medical needs.

The PT will supervise PTAs, coordinate care with the health team, document in medical records, and participate in professional activities. Travel to patient homes is required; mileage reimbursement and a sign-on bonus are offered.

Qualifications

• Effective interpersonal, communication, customer service and teamwork skills required.
• Licensed to practice in the Commonwealth of Virginia.

Responsibilities

• Provides patient care based on physician referral and age-appropriate assessment to achieve goals with patient/family.
• Coordinates patient care with therapists, Clinical Team Leaders, managers, physicians and other health care team members.
• Documents in the patient’s medical record and completes related forms per regulatory agencies and department policies.
• Supervises PTAs and collaborates with staff to deliver quality patient care.
• Participates in in-services, performance improvement, program development/evaluation, new employee orientation, committee membership and staff meetings.
• Delivers exceptional customer service and patient care with respect, dignity, kindness and empathy.
• Champions Carilion's mission, vision, and values and complies with policies and code of conduct.
