Job summary

Carilion Clinic is seeking a full-time Physical Therapist for Homecare in Radford, VA. You will assess patients, interpret physician referrals, plan and implement individualized treatment programs, and collaborate with a multidisciplinary team to ensure safe, effective care in the home setting.

Requires Virginia licensure, a PT degree (Bachelor, Master, or Doctorate), and BLS within six months. A sign-on bonus, mileage reimbursement, and equipment are offered with this role.

Qualifications

• Graduate of approved school of Physical Therapy.
• Licensed to practice in Virginia by the Virginia Board of Physical Therapy and eligible for licensure.

Responsibilities

• Provide patient care based on physician referrals with age-appropriate assessments and plan/implement treatment programs.
• Coordinate patient care and communicate with patients, families, therapists, managers and physicians.
• Document in the medical record and complete forms per regulatory and department policies.
• Supervise PTAs and collaborate with therapy staff to deliver quality care.
• Participate in in-services, performance improvement, program development, and meetings.
