Carilion Clinic seeks a Physical Therapist for its Home Health Float Team based in Roanoke, VA, providing coverage to surrounding areas. You will assess patients, interpret physician referrals, and design individualized treatment plans within regulatory guidelines for all ages and diagnoses.

The role involves supervising PTAs, collaborating with the therapy team, and documenting care in patient records. Travel to homes is required; mileage reimbursement is provided.

Formación

• Graduate of approved school of Physical Therapy.
• Licensed to practice in the Commonwealth of Virginia.

Responsabilidades

• Confirm and interpret physician referrals.
• Perform patient assessments and plan/implement individualized treatment programs.
• Coordinate patient care across the health care team.
• Supervise PTAs and collaborate with staff and students.
• Maintain documentation in the medical record per policy and regulation.
• Deliver high-quality patient care with empathy and professionalism.
