Trinity Health is seeking a part-time Physical Therapist to provide home/residence-based care. The role involves collaborating with a multidisciplinary team to create individualized treatment plans tailored to each client.
Responsibilities include performing initial assessments (OASIS where applicable), developing care plans with physician input, and communicating plan adjustments. The position emphasizes patient-centered goals and coordination with the care team.
Employment Type: Part time Shift: Day Shift Description: This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client’s home/residence.
Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client’s home setting.
Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.
Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.
Communicates with physician any requests to revise the plan of care.
Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.
Graduate of a university approved by American Physical Therapy Association
Licensed in the state of Iowa to practice physical therapy
Minimum of 1-year experience preferred
Community or home health experience desirable
