Trinity Health New Hampton Medical Center is seeking a Physical Therapist to join a day shift, full-time team, working in the client’s home to develop individualized treatment plans. The role emphasizes collaboration with a multidisciplinary team and physician input.

This position evaluates clients in home settings, performs initial assessments, and creates a plan of care with patient-centered goals while coordinating with the care team. Trinity Health is an Equal Opportunity Employer.

Formación

• Graduate of a university approved by American Physical Therapy Association.
• Licensed in the state of Iowa to practice physical therapy.
• Minimum of 1-year experience preferred.
• Community or home health experience desirable.

Responsabilidades

• Evaluates clients to obtain and interpret data for treatment planning in the client's home.
• Performs initial assessment/start of care services including OASIS assessment for PT clients when applicable.
• Develops an individualized plan of care involving the client, with physician collaboration.
• Communicates with physician any requests to revise the plan of care.
• Establishes patient-centered goals and collaborates with the interdisciplinary team.
