Job summary

Northwestern Medicine in Illinois seeks a licensed Physical Therapist to treat patients in the western suburbs within Home Health Care. You will apply diagnostic and therapeutic procedures, tailor plans of care, and document patient progress while upholding ethical standards and HIPAA rules.

You will collaborate with physicians, nurses and assistants, assess home safety, modify treatment as patients improve, and educate families on exercises and equipment use to support ongoing recovery and safe

Qualifications

• Licensed Physical Therapist registered in Illinois.
• Ability to communicate effectively with patients and the care team.
• Must be able to perform required lifting, bending and standing.

Responsibilities

• Evaluate patients’ level of functioning using clinical tests.
• Administer therapeutic exercises and modalities to improve muscle function.
• Instruct patients and families in home exercise programs and safety.
• Assess home environment for safety and provide modifications as needed.
• Document patient progress and communicate with physicians and nurses.
• Collaborate with the health care team to plan discharge and follow-up.
