Mercyhealth Visiting Nurses Association, Inc. is seeking a licensed Physical Therapist to provide home health services in Illinois/Wisconsin. You will evaluate patients, design and implement individualized treatment plans, and monitor progress across care settings.

The role emphasizes manual therapy, therapeutic exercises, and use of assistive devices, with family/caregiver education and coordination with physicians to ensure seamless care.

Qualifikationen

• Graduate of an accredited Bachelor, Master or Doctorate program for Physical Therapy.

Aufgaben

• Reviews Physician's referral, patient's condition and medical records to determine physical therapy treatment required.
• Obtains history and subjective components of the evaluation.
• Tests and measures patient's strength, balance, range of motion, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and records findings to develop or revise treatment programs.
• Plans and prepares written treatment plan based on evaluation of patient data.
• Administers manual exercises to improve and maintain function.
• Instructs, motivates, and assists patient to perform various physical activities and in use of assistant and supportive devices such as crutches, canes, and prostheses.
• Applies physical agents and massage modalities as part of treatment.
• Evaluates effects of treatment and adjusts plans to maximize benefit.
• Records treatment, response, and progress in chart or system.
• Instructs patient and family for home treatment procedures.
• Confers with Physician and other practitioners to coordinate care.
• Completes home health start of care, recertification, resumption of care, transfer and discharge assessments.
• Assists with supervisory visits and orientation of new partners.
