Northwestern Medicine is seeking a licensed Physical Therapist to provide in-home and clinic-based physical therapy in the western suburbs. The therapist aligns with NM's mission and adheres to ethical and compliance standards while delivering high-quality care.

You will evaluate patients, design individualized treatment plans, administer therapeutic exercises and modalities, train patients and families, and document progress for physician and care team.

Formación

• Licensed Physical Therapist in Illinois required.
• Strong written and verbal communication abilities.
• Ability to perform extensive lifting, bending, and standing.
• Home health experience preferred but not required.

Responsabilidades

• Evaluate patients to determine level of functioning and plan therapy.
• Administer therapeutic exercises and physical agents to improve function.
• Instruct and support patients and families in home and clinic settings.
• Document progress and communicate with physicians and care team.
