Job summary

Compassus is seeking a Home Health Physical Therapist in Birmingham, AL. You will provide coordinated care across age groups, plan and evaluate care plans, and perform clinical assessments including OASIS when appropriate.

The role requires an active Physical Therapist license, current CPR certification, and a preference for 2 years in acute care or rehab. You will work with an interdisciplinary team to ensure quality patient outcomes.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Two years of experience in an acute care or rehabilitation setting highly preferred.
• Active and unencumbered Physical Therapist license in state(s) of employment required.
• Current CPR certification required.

Responsibilities

• Provides coordinated home health care as a licensed Physical Therapist.
• Plans, implements, and evaluates patient care plans to restore or maintain well-being.
• Performs initial and ongoing clinical assessments including OASIS when appropriate.
• Documents progress notes and care plans; revises Plan of Care with physician input.
• Establishes transfer techniques and instructions for safe patient handling and equipment use.
• Collaborates with interdisciplinary team to coordinate services.
