Qualifications

• Bachelor of Science in Occupational Therapy required.
• At least 1 year of experience as an Occupational Therapist in acute care, home health, or rehabilitation setting highly preferred.

Responsibilities

• Participates in the development of occupational therapy plans of care for patients.
• Visits patients for assessment/evaluations and communicates with physician before start of care as needed.
• Completes OASIS assessments as appropriate.
• Implements the plans of care.
• Provides education and instruction to team members, patients, and families.
• Evaluates outcomes of care.
• Plans for discharge as appropriate.
• May participate in agency quality improvement programs.
• Supports growth and profitability via responsible resource use and educating the community.
• Participates in interdisciplinary team meetings to coordinate care.
• Applies agency policies and regulatory requirements in care.
• Completes required documentation and visit/coordination notes timely.
• Provides skilled restorative treatment to improve physical function and ADLs; designs orthotic/self-help devices and related training.
• Reports grievances and complaints to appropriate persons.
• Reports all accidents and incidents observed.
• Maintains proper documentation for billing.
• Supervises the care given by Occupational Therapy Assistants and Home Health Aides.
• Performs other duties as assigned.
