Compassus is seeking a Home Health Physical Therapist to provide coordinated care, plan and evaluate patient care plans, and collaborate with the interdisciplinary team to support patients of all ages in home health settings.

The ideal candidate holds a BS in Physical Therapy, an active license, CPR certification, and 2 years of acute care or rehab experience; strong math, communication, and documentation skills are essential.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Two (2) years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred.
• Active and unencumbered Physical Therapist license in state(s) of employment required. Current CPR certification required.

Responsabilités

• Plans, implements, and evaluates care plans to restore or maintain patient well‑being.
• Performs initial and ongoing clinical assessments including appropriate time points.
• Performs and oversees team members in patient care aligned with patient needs.
• Documents progress notes and orders; revises the Plan of Care as needed.
• Instructs patients in transfer techniques and safe equipment use.
