Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to assess and treat patients in the home, using modalities to improve mobility and independence. You will educate families and collaborate with the health-care team based on physician orders and the established plan of care.

You will participate in specialty programs such as Balance and Fall Prevention, Total Hip/Knee/Shoulder programs, and ensure care under the Branch Director's supervision. Valid state licensure is required.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must currently be licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and function.
• Instruct families and health-care team members as part of plan of care.
• Provide skilled services under physician orders and supervision of Branch Director.
• Implement specialty programs like Balance and Fall Prevention, Spine Safety, and joint replacement programs.
